Team brain injury: evaluation of a resource and service coordination model

Abstract:
"Team Brain Injury" or "Team BI" is a new approach to brain injury rehabilitation developed by the faculty of the Department of Physical Medicine and Rehabilitation to address a changing service delivery system characterized by shorter inpatient length of stays. Team BI seeks to manage the rehabilitation process from acute rehabilitation through long-term living based on a holistic view of the individual and using a coordinated team approach. Team BI educates patients and family via information customized to the needs and abilities of the learner, available in amounts and formats responsive to the stage of adaptation, and provided in the context of a long-term relationship. This demonstration project evaluated the Team BI approach and disseminated findings to the field.
