Examination of post-traumatic delirium and its impact on rehabilitation outcome and cost of care.
Abstract: The period of motoric agitation/withdrawal, disorientation, attention and memory deficits, mood lability, suspiciousness, and hallucinations that occurs after TBI has been called post-traumatic delirium (PTD) or post-traumatic amnesia (PTA). Recent research has indicated that confusion or attentional disturbance is the defining characteristic of this period rather than memory impairment. The present study seeks to determine the incidence of delirium in patients with TBI and to investigate the inter-relationships between PTD and PTA. Impact of urban vs. rural residence on outcome after TBI
Abstract: We predict that rural residents will be found to have poorer rehabilitation outcomes than urban residents. We predict that this difference will be partially predicted by time from injury to presentation to Model System emergency room, initial severity of injury, and early neurosurgical management. We also expect that the longer distances to obtain follow-up services, more missed appointments, and lower socioeconomic status will make additional contributions to predicting differences in rehabilitation outcome for rural vs. urban residents. Collaborative Study of Impaired Self-awareness after TBI
Abstract: The CSISATBI will conduct the first large-scale, prospective longitudinal study of impaired self-awarenessÂ’ (ISA) neural substrates, neuropsychological features, natural history, and relationship to functional and quality-of-life outcomes over the first year following moderate to severe TBI. In addition to several methodological innovations to improve interpretation of the quantitative data, the CSISATBI will provide the first systematic qualitative study of self-awareness from the perspective of persons with TBI and their families. Contributions of early impaired self-awareness and depression to predicting rehabilitation outcome and subjective well-being after TBI
Abstract: We predict that measures of impaired awareness and depression will make unique contributions to predicting outcome at discharge from inpatient rehabilitation and at one year follow-up. We also predict that impaired self-awareness and depression will both be common during in-patient rehabilitation and that patients will have more accurate self-awareness and be more depressed at one year follow-up. We will also investigate the relationship between cognitive functioning and impaired self-awareness. Concordance of Patient and Family Report of Neurobehavioral Symptoms at 1 year Post Traumatic Brain Injury
Abstract: Objective: To determine concordance between patient and family report of neurobehavioral symptoms and problems across 6 domains of function and 3 levels of injury severity at 1 year post traumatic brain injury (TBI).

Design: Prospective longitudinal design with follow-up between 10 and 14 months post injury.

Setting: 17 TBI Model System centers.

Patients: 267 adults with primarily moderate and severe TBI who had completed self-ratings and also had significant other ratings of neurobehavioral symptoms.

Relationship of early neuropsychological functioning with supervision needs one year after traumatic brain injury
Abstract: A recently completed study employing TBI Model Systems data revealed that cognitive deficits at one year after TBI are associated with a higher need for supervision at that time, among individuals who do not have significant motor deficits. The present study will examine the relationship between cognitive deficits early in the course of recovery (using the neuropsychological evaluation completed during inpatient rehabilitation) and supervision needs at one year after the trauma. The goal is to establish (if possible) guidelines for clinicians to follow in advising survivors and family members about potential supervision needs in the future.
