The Efficacy and Utility of a Community Transition Coordinator in Improving Community Integration, Quality of Life, and Return to Work: A Randomized, Controlled Clinical Trial.
Abstract: Survivors of TBI and their families are confronted with many new challenges. After being guided through many sudden life changes by trained professionals during inpatient rehabilitation, the transition to the community inevitably poses numerous obstacles that must often be faced with minimal outside assistance. This study investigates the utility of a community transition coordinator (CTC) assigned to adults with TBI in a randomized, controlled trial with the goal of developing a replicable model of community reintegration that provides linkages to services after discharge from comprehensive acute rehabilitation inpatient services.
